Your Community Star story will also feature a photo of you. Please upload your professional headshot or photo here no later than Thursday, September 15, 2022.

We ask that you please carefully follow the photo submission guidelines when selecting a photo to include for this publication. The publication will be offered in print and electronic format.

If you are familiar with the format of previous year’s Community Star profiles, please note that we’ve made changes to the 2022 program. We’re excited to offer Stars a few style options to choose from in this year’s new book format. Again, refer to the guidelines and sample page layout options for examples to help you select your photo submission type.

Please refer to our photo submission guidelines when choosing the best shot for your profile page. Share them with your photographer if scheduling a professional photo shoot. 

Submit Your Photographs

You can submit up to five high resolution photographs for the designer to choose from to lay out your page and for promotional materials.

Choose each photo separately below. They can be up to 10MB each in jpg, jpeg, or png file formats.

Each person, coalition, and organization nominated to be their state’s Community Star represents the faces and grassroots initiatives that are working to address the social determinants of health and improve the lives of those who call rural their home.
